7 North - Pediatric Hematology/Oncology - RN, Nurse Navigator

Responsibilities

Seeking a self-motivated individual who is interested in becoming part of a team dedicated to the care of children with hematologic/oncologic diagnoses. Time management, critical thinking, and excellent interpersonal communication skills are essential to this role. Flexibility and creativity are crucial in leading the care of the pediatric patients/families in a challenging and fast paced environment. Cross coverage to other general care units will also be required. The role serves as an adjunct to the multidisciplinary team partnering with unit Nursing staff, Providers, Consulting Services, Social Work and caregivers assisting in facilitating a patient focused plan of care aimed at ensuring a safe transition to discharge. Specific duties include but not limited to communicating directly with the patient's assigned nurse as they collaborate on creating and implementing actions to meet the patient's needs based on the plan of care and discharge needs. Facilitating multidisciplinary rounds, assisting with identification and resolution of barriers to discharge in collaboration with social work to facilitate timely discharge, assisting in oversight of patient education based on patient needs, discharge teaching with patients and their families, home care selection and referrals. Requires a collaborative working relationship with interdisciplinary care teams. Knowledge and experience in caring for pediatric heme/onc patients preferred but not required. 3 years relevant clinical experience desired. BSN required.

Qualifications

Requires a Diploma, Associate or Bachelor's degree from an accredited Nursing Program and an active New York State RN license in good standing. Applicants will have a minimum of 3 years of RN experience. This position utilizes expert skill and knowledge to deliver nursing care, directs others, and assumes the role as a care coordinator or preceptor.

Pay Range

Pay Range: $36.37 - $47.28

The referenced pay range represents the minimum and maximum annual salary for this faculty position. Individual salaries will be determined within the job’s salary range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations. Incentive-based compensation may also be applicable and will be discussed during the hiring process.
