Asst Dir, Health Home Program

GENERAL PURPOSE:

With minimal oversight, the Assistant Director, Health Home Program will manage the daily operations and expansion of UR Medicine Embedded Health Home Care Management Program. The UR Medicine Embedded Health Home Care Management Program is comprised of three service lines (embedded care management, central care management, and perinatal care management) that spans six primary care locations, 2 hospitals, and ambulatory settings across the URMC enterprise. This role is responsible for the direction and management of all three, two of which are in an aggressive expansion phase. Because of this complexity, a successful leader will have a developed understanding of clinical and administrative processes, the ability to manage multi¿faceted and complex programs, projects and people across a variety of settings, departments, and service lines. In addition to the daily operations and expansion, the Assistant Director role is integral in promoting the strategic alignment of care management services across the enterprise by collaborating with leaders to develop a robust and comprehensive infrastructure to support the health system’s mission, operations, and growth. Experience managing people at all levels, proven success with building consensus across multiple stakeholders in complex environments is a must.

JOB DUTIES AND RESPONSIBILITIES:

  • Provide expert-level knowledge of health home care management policies, procedures, and regulatory requirements, as well as clinical and eRecord workflows of care management. Responsible for providing interpretation of this information and interact with stakeholders to create understanding among stakeholders of various levels of engagement (front-line staff to leaders across the health system). Responsible for developing strategies for operational success and leading the change management of same.

  • Direct Embedded Health Home team members by creating an inclusive environment and holding a team to excellence by providing mentoring, professional development, and supervision to direct reports (5) and leadership to indirect reports (up to 40). Responsible for recruiting and hiring, managing employee performance, and completing performance evaluations of direct reports.

  • Manage cross-entity Health Home projects including projects with significant information systems integration; Provide data-driven evidence of health home operations across the embedded care management program to SMH Leadership and Department Administrators, including preparing and delivering quarterly reports for each of the 6 primary care practices. Identify and implement system / program changes in workforce and resources to maximize client capacity; Maintain alignment in referral management, financial reconciliation, and reporting.

  • Work with leaders across the UR Medicine Enterprise to develop and then take responsibility for the execution of strategic plans to optimize and expand the program to meet the needs of stakeholders and institutions. (Including ACOs, UR Medicine Hospitals, Primary Care Practices, and URMC/AHP’s Care Management Organization and other stakeholders)

  • Represent UR Medicine’s Health Home Program to external NYS Health Home administrative bodies.Serve as a liaison to external stakeholders to communicate, advocate, and demonstrate capacity for collaboration and refinement of health home operations to serve the UR Medicine and regional patient population.

  • Collaborate with other internal and external care management leaders and other stakeholders to execute the vision to capitalize on opportunities to collectively leverage shared resources and initiatives. This includes the provision and communication of data-driven evidence of health home operations, with attention to assessment and continuous process and quality improvement.

REQUIREMENTS:

  • Bachelor's degree in related field required; Graduate degree in Business, Public Health, Social Work or Health Administration preferred.

  • Public Health, Social Work or Health Administration preferred.

  • 5 years of healthcare management level experience, including leading people/teams; or an equivalent combination of education and experience;

  • Previous work experience in clinical setting, population health initiatives or care management required;

  • Strong project management skills, including organization of individual and team work; prioritization; problem solving; adaptability; flexibility; and attention to detail

  • Teamwork, including the ability to draw out diverse perspectives, consider problem resolution from multiple angles, and build upon the experiences of all team members to create solutions that are broadly supported

  • Exceptional judgment and ability to learn the needs of different components of the UR Medicine Enterprise

  • Excellent verbal and written communication and interpersonal skills

  • Proficiency with Microsoft Office programs (Outlook, Word, Excel, Access and PowerPoint) and ability to learn new platforms as needed (Netsmart, eRecord)

Pay Range

Pay Range: $95,306 - $142,979 Annually

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
