Responsibilities
Preparing Interview Forms 20%
Retrieve birth notices
Check worksheets and see what information is needed for completion
Log book entry
Interviews 30%
Verify worksheet information and fill in missing information
For each single mom, explain the Acknowledgment of Paternity
Explain various forms and get required signatures
Chart Abstractions 20%
- Gather required medical information as set by NYSDOH from each mother’s medical record
Data Base Entry 20%
- Enter birth certificate data from worksheets into the NYSDOH web site
Miscellaneous 10%
Must have a pleasant attitude, must be sensitive to difficult situations and work cooperatively with others in the office and hospital staff
Weekend coverage (4 hrs.) to be rotated equally among staff
Routine filing and coping of forms, letters, etc
Answer numerous phone calls from new mother’s regarding birth certificates and Social Security numbers
