The part-time BHC Social Worker is responsible for the recruitment, and training and ongoing supervision of assigned BHC Outreach Workers. The part-time BHC Social Worker is part of the BHC leadership team, which communicates regularly to discuss all aspects of program implementation and evaluation, funding updates, clinical concerns, and the integration of the Strong Outreach and Mt. Hope teams. The part-time BHC Social Worker regularly reviews medical records for all assigned children and mothers and communicates all medical needs, appointments and concerns back to appropriate members of the BHC team for collaboration and follow-up. The part-time BHC Social Worker will liaison with the pediatric and OB/GYN health care team as part of the practice’s interdisciplinary patient care approach. Lastly the part-time BHC Social Worker is responsible for collecting and entering outcome data and keeping the appropriate databases up-to-date.
The part-time BHC Social Worker oversees case management services involving referral and case coordination with the collaborating BHC service providers, and other health and social services agencies. 45%
The part-time BHC Social Worker consults with interdisciplinary team members within BHC and participant medical homes.
The part-time BHC Social Worker (a) participates in the development of protocols, tools and professional development that guide program service delivery for achievement of quality and accountability measures; (b) documents summaries of service plans in the patients medical record; (c) maintains data on program utilization, productivity and effectiveness and assists BHC Clinical Coordinator with preparing program reports; (d) liaisons with BHC collaborating partners - the Mount Hope Family Center, the Monroe County Division of Social Services and the United Way. 25%
The part-time BHC Social Worker participates in activities of the Social Work Division including but not limited to attendance at staff meetings, completion of time reporting, mandatory in-service and ongoing in-service education, and UHS reviews. 5%
The part-time BHC Social Worker may serve on Social Work Division committees, community coalitions and advisory boards.
Supervision and Teaching:
The part-time BHC Social Worker recruits, trains, supervises and evaluates the performance of three BHC Outreach Workers assigned to this position. The BHC Social Worker performs personnel management activities when indicated. 25%
The part-time BHC Social Worker may be involved in clinical teaching of multidisciplinary staff and trainees, including social work students.
Professional Accountability, Supervision and Evaluation
Clinical and Administrative Accountability:
The part-time BHC Social Worker is administratively and clinically accountable to the Associate Director of the Social Work Division for provision of services and adherence to Social Work policies, procedures and standards of professional conduct.
Evaluation, Supervision and Performance:
The part-time BHC Social Worker is clinically and professionally supervised by the Associate Director of the Social Work Division by whom the professional performance evaluation is done.
Annual salary and professional promotion recommendations are determined by the Associate Director of the Social Work Division in consultation with the Director of Social Work and Patient & Family Services.
The part-time BHC Social Worker must have a Master of Social Work from an accredited institution, be licensed in New York State within 18 months of hire and have two post-graduate years of relevant experience. Experience in supervision preferred.
