Responsibilities
Overview
Serve as Director for Golisano Children’s Hospital Care Management, a care coordination and case management program that provides interventions to assist high-risk patients with overcoming social determinants of health to improve health outcomes for patients in the Finger Lakes region. This position will continue to oversee all Medical Home care coordinators in Department of Pediatrics General Pediatric high-risk clinic. This Administrator will manage all operational and financial functions of the Health Home including capital and operating budgets, workforce development, call center functions, referral management, contracts, space, equipment maintenance, care management, and outreach functions. As Director will provide supervision and evaluations of Call Center staff, Medical Home Care Coordinators, and other health home staff. The position will work collaboratively with the E-record build team to develop care management tools to support programs in delivering exceptional patient outcomes. Will serve as Chief Liaison to all internal and external care management committees, agencies and officials including DSRIP, NCQA PCMH, GRHHN, CHHUNY, and others as needed. Major Responsibilities
1. Operations/Space (25%)
a. Ensures the administrative structure, infrastructure, and work processes adhere to Health Home standards.
b. Ensures success with internal mission and specific performance metrics, analyzing trends.
c. Establishes program policies to meet operational needs and implements as needed
d. Oversee space matters including space, furniture, and equipment assignments for non-clinical activities.
e. Ensures success with NYS Health Home expected outcomes
f. Reviews and negotiates contracts for Children’s Health Home with NYS agencies (i.e., Hillside). Ensures contracts do not expire.
g. Monitors all contracts to ensure contractual obligations are met. Identifies and resolves issues.
2. Finance (30%)
a. Prepares operating and capital budgets; reviews and identifies variances. Develops plan of action to resolve variances as needed. Prepares budget modifications as needed.
b. Provides oversight of all account monitoring
c. Supervises the coordination and review of reports to external agencies and officials
d. Approves expenditures in accordance with budgets
e. Ensures all contractual services are performed, billed, and accounted for correctly
f. Collaborates with Medicaid Managed Care plans for compliance measures and fiscal sustainability
g. Prepares business plans for incremental resources
h. Provides oversight of all account monitoring
i. Supervise coordination of quarterly reports or other reports based on contractual obligations
3. Human Resources – Serve as the primary supervisor for all nonclinical staff (20%)
a. Staff recruitment, training, assignments, and evaluations
b. Monitor all internal regulatory and external HR / faculty requirements (HIPAA, health assessments, other mandatories)
c. Establish and maintain environment conducive to effective communication and problem resolution
d. Processes HR paperwork for staff and monitors the time records for HRMS
e. Counsel staff, acting on matters of discipline, promotion, salary and other personnel issues
f. Coordinate and review all requests for new and replacement positions
4. Information Technology/Data Management (15%)
a. Monitors quality indicator dashboards and reports to ensure compliance with quality measures for National Committee for Quality Assurance (NCQA) Medical Home Accreditation and NYS Health Home requirements
b. Designs and develops infrastructure for care management tools in electronic health record systems working in conjunction with hospital IT staff
c. Provides data reports and maintains integrity of data as required for program initiatives. Reviews all reports to ensure accuracy prior to agency submissions based on contractual requirements.
e. Evaluates and implements all IT infrastructure needs to support functions
5. Marketing and Communications (5%)
a. In conjunction with Chief Administrative Officer oversees the coordination of mission requested marketing and public relations communications including Health Home annual report, brochure, website, and community correspondence.
b. Oversees all consumer advocacy interactions and program communications
6. Other duties as assigned. (5%)
Requirements
Bachelor’s degree. A Master’s degree, equivalent course work, or specialized experience in an appropriate field related to the assignment is desirable. Five years relevant experience including at least two years at a managerial level.
Demonstrated leader with progressive experience as an administrator that incorporates a global perspective, along with a commitment to diversity and ability to work in a team setting with all levels of management, faculty, and staff. Ability to develop business proposals, manage budgets, analyze trends, and staff efficiency; excellent oral and written communication skills; Superior knowledge of business planning, data management, care management, quality improvement, and productivity benchmarks. Local and regional travel required. Must have reliable/efficient transportation.

Health Home Call Center Staff, Medical Home Care Coordinators, and other non-clinical staff.
