GI Nurse Case Manager - Advanced Sub-Specialty - Registered Nurse, Level III

Responsibilities
URMC GI Practice is in search of a driven, team-oriented nurse to join our multidisciplinary team providing focused, sub-specialty care to our Advanced GI (biliary/pancreatic disease) patient population. This role provides nurse case management for patients with complex medical needs in management of Pancreatic cancer, bile duct dysfunction, high grade dysplasia management, and many more. Nurse Case Manager will develop and implement a care management plan based on patient/provider developed goals and objectives to promote improved health care outcomes and quality of life. This role involves assisting with the referral process, monitoring the need for stent replacement/removal, providing education and support, and telephone triage/ in-basket management. The ideal candidate will have a Bachelor’s degree and 2-3 years GI nursing experience with strong organizational and communication skills as well as experience working within the electronic medical record. The nurse case manager will work effectively as a member of the health care team by collaborating on population health management, complex care and transitional care management.
Qualifications
Requires a Diploma, Associate or Bachelor's degree from an accredited Nursing Program and an active New York State RN license in good standing. Applicants will have a minimum of 3 years of RN experience. This position utilizes expert skill and knowledge to deliver nursing care, directs others, and assumes the role as a care coordinator or preceptor.
