Home Infusion Nursing Care Team Coordinator

POSITION SUMMARY:
Responsible to provide clerical and administrative support for patient and staff schedules. Provides scheduling support for home infusion nursing to ensure home nursing agencies have covered patient visits including chemotherapy takedown. Communicates visit times to home nursing agencies.
TYPICAL DUTIES:
Coordinates chemotherapy disconnect times from UR infusion centers with home infusion, and other home nursing agencies responsible for patient care.
Proactively schedules and plans unassigned visits on the home infusion calendar.
Prints delivery tickets for routine visits for home infusion patients.
Communicates cancelled chemotherapy visits with nursing agencies and providers and manages delivery tickets for cancelled visits.
Assists home infusion nursing intake specialist with referral process
Provides clerical support to home infusion team: paperwork, scanning, delivery ticket confirmation, compiles start of care folders for patients
Communicates home infusion needs to home nursing agencies for scheduling purposes including day/time needed. Completes documentation in home infusion medical record.
Receives nursing administration notes from nursing agencies for pharmacy and scans into home infusion medical record
QUALIFICATIONS:
2 years of post-high school education in secretarial studies or the equivalent required
3 years of secretarial and office experience, preferably in a community health agency or medical setting required
Knowledge of medical terminology preferred

Pay Range
Pay Range: $ 17.64 - $ 24.71 Hourly
The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
