Liver Transplant Coordinator - Post Transplant - Registered Nurse, Level IV

Responsibilities

Exciting position in the solid organ transplant program supporting our transplant patients by providing them with the care and resources they need in the post-transplant phase of care. Establishing good rapport with patients, their support team and outside providers is key to this position. The ability to work in a fast paced environment while juggling multiple responsibilities is paramount. Strong clinical skills and an interest in patient education is required. This is a detail-oriented position that requires a great deal of time on the phone working with patients and their support team, answering questions, assessing patient status and providing guidance while pulling in other multidisciplinary team members when needed. Candidate must have excellent communication skills and be comfortable working in a multidisciplinary setting. Position requires involvement in quality committee meetings and projects. Participation in developing and maintaining policy and procedures is encouraged. The candidate will serve as a resource to internal and external health care providers.

Qualifications

The coordinator is responsible for adherence to hospital and nursing policies, procedures and practice standards, education of patients and staff, assurance of service excellence, completion of state and hospital required reviews of practice and participation in performance and quality improvement and other activities as required.

Pay Range

Pay Range: $45.10 - $67.64

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.

Full/Part Time:
Schedule: FT position available M-F, with some rotating overnight call taken from home managing the logistics of setting up liver transplants and addressing patient phone calls.
