Pediatric Nursing - Division of Pediatric Hematology/Oncology - Nurse Navigator

Responsibilities

This pediatric outpatient position combines acute care skills with care management and effective communication, to best care for our pediatric patients with Sickle Cell Disease and Vascular Anomalies. Some of the Nurse Navigator responsibilities will include patient triage, phone and MyChart communications, patient and caregiver education about Sickle Cell Disease and Vascular Anomalies and contributing to a healthy team environment. This role also includes tracking and evaluating barriers to care to support ongoing health maintenance needs, appointment attendance, and transition to adult care.

Qualifications

Pay Range

Pay Range: $36.37 - $47.28

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
