General Purpose:
The Center for Community Health & Prevention (CCHP) at the University of Rochester Medical Center (URMC) has recently been awarded a five year grant by the NYS Dept. of Health. This grant, Peer Education, Outreach and Shared Decision Making for Persons at High Risk for Prostate Cancer (PCPEER) , supports peer education and outreach, personalized coaching, linkage to community services to address barriers to healthcare and referral to health care providers for prostate cancer screening.
In accordance with the PCPEER Grant, the Community Liaison and Peer Educator (CL&PE) will lead and deliver all grant activities which will be directed to Black men, ages 45-69 years old, who bear a disproportionately high burden of prostate cancer in Monroe County. Programming will address the risks and benefits of prostate cancer screening and will provide support to engage program participants in shared decision making (SDM) about prostate cancer screening with a health care provider. It is vital that the CL&PE has demonstrated cultural competence in serving the communities the PCPEER program is targeted to serve.
The start date for this position is October 1, 2023.
The CL&PE will represent the PCPEER grant and the Center for Community Health & Prevention with partners, providers and patients and function in multiple capacities to meet all performance measures and program goals of the grant.
With latitude for independent judgement, the CL&PE will report to the Sr. Administrator and work closely with the Director of Community Health Partnerships (DCHP), Manager of the Cancer Services Program of the Finger Lakes Region (CSP-FLR) and Wilmot Cancer Institute’s Community Outreach and Education team (COE.)
This community facing role offers limited options for remote work.To meet the goals of the program, some evening and weekend hours are anticipated.
Responsibilities:
Meet and educate community partners about program services by attending community health-related talks and events, and networking with those in attendance to increase awareness of the PCPEER and cancer prevention programs. Participate on community coalitions and establish and cultivate relationships with individuals and community-based organizations for partnership opportunities. Attend Cancer Community Action Council (CCAC) meetings
Meet and educate primary care providers about program services, assess the availability of case managers, patient navigators and other staff in the practices. Establish referral agreements with primary care practices to refer men for Shared Decision Making for prostate cancer screening.
Perform data entry and database management activities, enter program data accurately and timely, participate in evaluation design and resource allocation, monitor progress, and meet multiple deadlines on time. The CL&PE will act as an ambassador of PCPEER, CCHP and Wilmot COE programs, identifying new opportunities for engagement and/or growth or expansion of programmatic offerings. Work closely with CSP-FLR for strategic engagement.
Work collaboratively with CSP-FLR, Wilmot Cancer Institute, CCHP to increase access and promote cancer screening programs and prevention resources.

Qualifications:
Required:
Bachelor’s degree with major course work in an appropriate health, social or technical field required.
2-3 years of experience or an equivalent combination of education and experience required.
Preferred:
Experience in nutrition, physical activity, or cancer prevention education a plus.
Ability to interact effectively with a wide range of individuals, from varying cultural, ethnic, socio-economic and educational levels.
Uses computer programs proficiently, including use of the Internet, Microsoft Word and Excel, and PowerPoint, database management systems, and data entry programs.
Ensures the security and confidentiality of all data per HIPAA, PHI and IRB guidelines.
Excellent interpersonal, presentation, and communication skills, including written communication.
Ability to work independently and as part of a team.
Excellent customer service, organizational and time management skills.
Some weekends and evenings are required.
Ability and willingness to travel.
Employment with the grant will end September 30, 2028, unless extended.

Pay Range
Pay Range: $ 41,746 - $ 56,347 Annually
The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
