Pharmacist Inpatient - Clin II (Wilmot Cancer Center)

GENERAL PURPOSE:

The Oncology Pharmacist position plays a pivotal role in our healthcare team, specializing in both infusion center and acute care inpatient services. This role involves the precise management of cancer treatment plans, ensuring the safety and effectiveness of medication regimens for patients. The expertise in this position directly impacts the quality of care provided, emphasizing the critical nature of this role in delivering optimal outcomes and support to cancer patients within the URMC healthcare network.

SPECIFIC RESPONSIBILITIES :

Medication Management:

  • Operating within an oncology pharmacy that supports both outpatient infusion centers and inpatient acute care oncology units, while harnessing the power of EPIC® (Electronic Health Record) integration

  • Review and verify medication orders for accuracy, appropriateness, and potential drug interactions.

  • Prepare and dispense chemotherapy drugs and other medications used in cancer treatment.

  • Calculate drug dosages based on patient-specific factors such as weight, height, and organ function.

  • Collaborate with healthcare professionals to develop individualized treatment plans and adjust medications as needed.

  • Participate on an oncology treatment plan committee to aid in upkeep of EPIC® Beacon treatment plans and creation of new treatment plans.

  • Assess and develop selection of evidence-based and therapeutically appropriate anticancer therapies based on diagnosis, performance status, organ function, disease staging and pertinent pharmacogenomic status.

  • Monitor patient responses to therapy, assess adverse effects, and provide appropriate interventions.

Collaboration and Consultation:

  • Work closely with oncologists, nurses, and other healthcare team members to optimize patient care and outcomes.

  • Review primary literature resources and appropriate guidelines (e.g., NCCN, ASCO) when evaluating, recommending or implementing anticancer therapy and supportive care

  • Prospectively identify and triage chemotherapy and supportive medication issues such as dose adjustments, prior authorizations, etc.

  • Participate in multidisciplinary oncology rounds and meetings to discuss patient cases, treatment plans, and medication-related issues.

  • Provide evidence-based recommendations on drug selection, dosing, and supportive care for patients with complex medical conditions.

Support and Mentorship:

  • Identify opportunities to assist and mentor pharmacy technicians in the compounding of chemotherapy medications.

  • Provide guidance and oversight to ensure the accurate preparation and handling of hazardous and non-hazardous drugs, adhering to established safety protocols.

  • Serve as point person for pharmacy technicians on proper aseptic techniques, equipment usage, and quality assurance measures.

  • Foster a collaborative and supportive environment to enhance the skills and knowledge of pharmacy technicians in oncology compounding.

  • Assist in precepting pharmacy students and residents in oncology practice.

Patient Education and Counseling:

  • Educate patients and their caregivers about chemotherapy medications, treatment schedules, and potential side effects for therapy such as but not limited to R-CHOP, EPOCH, PR-CHP

  • Provide guidance on proper medication administration techniques and strategies for managing treatment-related symptoms.

  • Address patient concerns, answer medication-related questions, and ensure understanding of treatment regimen.

Quality Assurance and Compliance:

  • Ensure compliance with institutional policies, state and federal regulations, and industry guidelines related to oncology pharmacy practice such as but not limited to USP 797 and USP 800

  • Perform medication safety activities, including medication error reporting via RL solutions, root cause analysis, and implementation of preventive measures.

  • Stay updated with advances in oncology pharmacy practice through continuous professional development and participation in relevant educational activities.

Research and Quality Improvement:

  • Contribute to research initiatives and quality improvement projects related to oncology pharmacy practice.

  • Collect and analyze data to evaluate medication-related outcomes, identify areas for improvement, and implement evidence-based interventions.

  • Present new medications to Oncology Therapeutics subcommittee.

QUALIFICATIONS :

  • Bachelor of Science degree in Pharmacy or Pharm D required.

  • Experience in acute care pharmacy or completion of a Post Graduate Year 1 Pharmacy Residency preferred.

  • Training in hospital pharmacy practice, including internship preferred.

  • New York State Pharmacy License or eligibility required.

Pay Range

Pay Range: $121,202 - $149,968 Annually

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
