Responsibilities

GENERAL PURPOSE:

This is an analytical position responsible for practice level patient-centered medical home (PCMH) reports and population health management through tracking/monitoring, reporting and outreach. Directly supports operational efforts for all population management initiatives that involve quality metrics; assists practice teams in quality and process improvement initiatives. Support clinical teams and care management team to optimize patient outcomes. Utilize electronic registries such as Regional Health Information Systems, Arcadia Analytics, and eRecord dashboards for comprehensive patient care management. Coordinates internal and external referrals accordingly.

RESPONSIBILITIES:

Provider Support:

  • Acts as liaison to provide assistance, exercise discretion and use judgement in stressful situations with patients and caregivers in various aspects such as: coordination of appointments and referrals.

  • Provides administrative support to faculty, nurses and other providers at the Complex Care Center. Works closely with departmental staff and outside vendors to ensure success.

  • Communicate with patients as needed on any gaps that must be closed before the patient arrives in the office (such as lab work/testing.) and document accordingly.

  • Researches hospital medical records for information requested by physicians, insurance companies and attorneys. Finds missing orders and pends them for review and approval.

  • Manages provider(s) documentation and information. Follows workflows set in place to satisfy requirements of a Patient Centered Medical Home

  • Participate in daily “huddle” with physicians and staff to communicate patient needs and facilitate team collaboration. Participates in discussions with RN care manager of patients on ways to improve metric gap closure.

    Referral Management:

  • Main contact for ensuring all referrals are completed in a timely manner and that follow up is completed.

  • Gathers all necessary information for referrals for insurance companies.

  • Makes sure that patient is aware of the referral and assist in making appointment.

  • Tracks the entire referral process from placement to completion.

  • Follows up to confirm all orders are placed correctly in eRecord for the procedure or testing that has been ordered. Is accountable for completeness of all relevant information.

  • Follows up with patient and ensures appointments for referrals were completed

  • Prepares and reports metrics regarding referral management

Data/ Report Acquisition and Validation:

  • Run eRecord reports and update patient charts accordingly (ie: HMT, vaccines, etc.).

  • Utilize available templates in the eRecord reporting library to create custom-crafted reports per practice’s specifications.

  • Performs complex analyses on the validated datasets based on pre-defined information needs.

  • Custom-create data extraction reports in EPIC and outreach to patients as appropriate.

  • Use Excel and other database platforms to create spreadsheets and pivot tables to track patient outreach and metrics where required. Highlight data patterns and trends and disseminate data to lead data coordinator.

  • Mine for completed PCMH, other incentive plan metrics using the health maintenance audit

Reception:

  • Provides cross coverage when Outpatient Access Specialist is not available

Administrative Support: (10%)

  • Responsible for training and coordinating activities of OAS.

Other duties as assigned

QUALIFICATIONS:

  • Associate’s degree in Medical, Secretarial or related field and a minimum of one-three years of relevant experience required; or an equivalent combination of education and experience. Medical Terminology, experiences with surgical/appointment scheduling software (such as Flowcast), and electronic medical records, preferred. Demonstrated customer relations skills. Required/Preferred

  • 1-3 years of related work experience required

  • Ability to work effectively with individuals across all job levels. Flexible in approach and appreciates the demands of a fast-paced, constantly evolving division preferred.
