Responsibilities
Position Overview:
The Quality & Patient Safety Specialist will be assigned to The University of Rochester Primary Care Network (PCN) which consists of 42 practice locations and 300+ providers. The PCN extends from Syracuse to Buffalo and as far south to the Pennsylvania border. The Quality & Patient Safety Specialist will support the implementation of clinical and patient safety initiatives and spreading of best practices across the PCN.
Essential Functions:
Collaborates with designated leaders in the development and execution of quality, patient safety and performance improvement initiatives
Ensures accuracy of data through appropriate data validation processes
Works with the Information Services Department in designing and trouble-shooting reports and databases
Analyzes data to identify opportunities for improvement; works with providers and staff to improve performance and outcomes
Creates reports and delivers results to providers, PCN leadership, committees, and others
Acts as a change agent, in a collaborative manner, through the analysis and effective use of data and other quality improvement tools (e.g., process mapping)
Contributes toward the development of a culture of safety by promoting teamwork and effective communication; promoting and reinforcing safety practices within the offices; and working with others in a nonjudgmental manner
Contributes toward a learning environment by developing and/or delivering education programs on patient safety; disseminating patient safety information to staff and providers; sharing best practices and lessons learned
Conducts periodic audits to assess ongoing status of initiatives
Facilitates prioritization of improvement activities with PCN leadership
Navigates the organization to address and work through barriers, escalating problems to leadership when appropriate
Minimum Qualifications:
Associate Degree, preferably in a healthcare related field
Clinical/operational experience in healthcare, preferably in an ambulatory care setting
Experience as a data analyst or equivalent, preferably working with healthcare data/informatics
Proficient in desktop applications and database programs; particularly, MS Word, Excel, PowerPoint (graphic development and presentations is highly desirable)
Working knowledge of health information technology/electronic health records, preferably the Epic platform
Knowledgeable of quality, patient safety, performance improvement principles, methods, and tools

Qualifications:
Bachelor's degree with major course work in business, healthcare or technical field; or an equivalent combination of education and experience.
Preferred Qualifications:
Experience facilitating healthcare quality, patient safety or performance improvement initiatives
Experience with delivering presentations to a variety of audiences
Two years’ project management experience and up to 5 years increasing responsibility in health care areas of quality and data analytics.
