Responsibilities

Position Summary:

The social work assistant is responsible for the implementation of discharge plans for adult patients aged 18 and older and for ensuring continuity of care upon discharge from the Adult Partial Hospitalization Program (PHP). The social work assistant participates in the planning and delivery of appropriate service linkages, and serves as liaison and advocate for patients and their families with identified service providers. The social work assistant meets with patients and families to assist with the collection of psychosocial data and collaborates in the evaluation and discharge planning processes. The social work assistant participates on the interdisciplinary treatment team for the patients on his/her caseload. The social work assistant, in collaboration with licensed clinical staff, participates in crisis intervention, maintenance of the therapeutic milieu, and assessment and management of patient safety. The social work assistant serves as a consultant to members of the interdisciplinary team regarding child abuse, domestic violence, and entitlement programs. The social work assistant may also provide psycho­educational group services in the PHP. The social work assistant develops and maintains relationships with community agencies and providers who refer to and receive patients from the PHP.

The social work assistant functions as the Discharge Planning and Resource Coordinator for the PHP. The social work assistant will utilize her/his discharge planning expertise and extensive knowledge of critical resources. The social work assistant may provide oversight, supervision, and consultation for other PHP BSW staff.

Responsibilities:

Bachelor's degree in Social Work; or an equivalent combination of education and experience. Clinical or Community Social Work experience in health setting preferred.
