The Social Work Assistant is an integral member of the Employer Based Services teams, which includes Behavioral Health Partners (BHP) and UR Medicine EAP. In their role at BHP, she/he is responsible for the implementation of phone triage and psychosocial assistance for patients aged 18 yrs. and older who are seeking care with BHP. They participate in the planning and delivery of appropriate service linkages, and serves as liaison and advocate for patients and their families with identified service providers. The SW Assistant will at times identify patients with additional needs and will in such cases provide the most appropriate initial and follow up interventions to complement what is already being provided by other members of the psychiatric team. Such interventions might include, for example, referrals to specialty psychiatric and substance use services, educational sessions, case management, condition management and/or brief evidence based interventions focusing on behavioral activation, problem solving interventions and/or motivational strategies to further the care of the patient. The position will require collaboration with physicians, advanced practice providers, nurses, psychotherapists, administrators and other staff. General responsibilities in their role at EAP include triaging crisis phone calls and requests for critical incident responses. This role also includes participating in events that promote BHP and EAP services.

  • Administrative ( 5%)

  • Ensures that Hospital, Department, and Social Work Division administrative and clinical policies and procedures are implemented with regard to patient care, discharge planning and professional responsibility.

  • Participates in activities of the Collaborative Care and Social Work Divisions as appropriate; including completion of time reporting and other Hospital or Division forms, Hospital and Division mandatory in-service training programs.

  • Attends Social Work Division and Program Staff meetings as appropriate.

  • Complies with Social Work Division and Departmental quality assurance and staff development activities.

  • Subscribes to the Departmental goal of creating a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences.

  • Participates in available cultural competence events and training appropriate to job duties.

  • Clinical in BHP service ( 75%)

  • Conducts telephone screening on new referrals to ensure appropriateness for clinic services.

  • Coordinate the care of existing patients by facilitating communication between different providers of care, hospital services and referral sources.

  • May assist with screening for Depression, Anxiety, Stress and social functioning. Provides feedback to other clinicians at BHP about patient's improvement in care, length of stay, and facilitates transfer to specialty mental health or substance use treatment programs as clinically indicated.

  • May provide case management services to patients of the BHP clinic.

  • May provide outreach and education to individuals eligible for services at BHP including being present at promotional events, participating in new staff orientation and contributing to the BHP newsletter and blog.

  • May provide outreach to recently discharged patients to ensure continued stability and to invite back into treatment as necessary.

  • Monitors requests for patient records and facilitates completion of medication prior authorizations, disability and other forms required as part of the patient's ongoing care.

  • Documents social work interventions in the patient's chart in compliance with Division, Department and Hospital policy.

  • May provide direct care services as well as telehealth appointments to patients including educational sessions, case management, condition management and/or brief evidence based interventions focusing on behavioral activation, problem solving interventions and/or motivational strategies to further the care of the patient and complement other clinical services they might already be receiving by other members of the BHP team.

  • Clinical in EAP services (20%)

  • Triage crisis phone calls during regular work hours, and determine if the individual calling needs to be scheduled into a crisis slot appointment, assist with screening for self-harm, assist in connecting individual with emergency services if needed.

  • Teaching

  • The social work assistant may function as a field work instructor for undergraduate social work students. He/she may provide education to medical students regarding the role of social work and resources available in the community.

  • Research

The social work assistant may participate in social work and/or interdisciplinary research endeavors, as determined by the Social Work Supervisor in consultation with Clinical Manager in Psychiatry.


  • Supervision/Consultation

The social work assistant's professional performance is supervised by a Social Work Division supervisor who is licensed by the State of New York.

  • Accountability

  • The social worker is accountable administratively and clinically to his/her social work supervisor (and through him/her to the Social Work Division Clinical Manager in Psychiatry) for provision of social work services, and adherence to Social Work policies, procedures and standards of professional conduct. The social worker's job performance must conform to the expectations set forth in the position description .

  • The social work assistant is accountable administratively and clinically to the to the Director of Behavioral Health Partners for providing professional social work services in accordance with the policies and standards established by the Department of Psychiatry and BHP.

  • Evaluation

Social work evaluations are conducted annually by the social worker assistant's supervisor, with commentary from the Director of BHP. Annual salary and promotional recommendations are made by the social worker assistant's

Supervisor in consultation with appropriate Social Work Division personnel. Salary increases and promotional recommendations are subject to guidelines established by the Social Work Division, Strong Memorial Hospital and the University of Rochester.


The social work assistant who occupies this position must have a BSW from an accredited social work undergraduate program.


This position description is reviewed annually by the social worker and his/her supervisor. Position review provides opportunity for comment from the Director of BHP, and for updating, as needed. Similarly, if a new social work assistant is hired for the position, a review and update of the position description should occur. Changes in the position description are recommended by the supervisor, in consultation with the Director of BHP. Any position description modification is authorized by the Social Work Division Clinical Manager for Psychiatry.
