Social Worker / CARES Center

GENERAL PURPOSE:
The Social Worker is responsible for providing clinical services to children ages 0-21, followed by the CARES Pediatric Complex Care Program. Services are also offered to the families of these patients. Services are delivered via telemedicine, eRecord platforms, telephonic, and in-person at hospital visits and home visitation. The social worker works autonomously and collaboratively with a variety of health care and community providers to optimizethe patients’ overall health. Focus is on a seamless and well coordinated model that supports the patient and care provider. The Social Worker also serves as a health care resource, interdisciplinary consultant, and patient advocate.
RESPONSIBILITIES:
Completes psychosocial assessments and based on assessments provides social work treatment and supportive counseling to patients and families enrolled in the CARES Pediatric Complex Care Program.This includes providing information and linkage for patient and families around special Medicaid programs, for supplemental medical insurance, and medical care at home. Also includes identifying and managing all reports of suspected child abuse and neglect in the identified patient population and other psychosocial rsiks.
Provides concrete services to families needing assistance with transportation and meals during the outpatient visits and assistance to patients and families which facilitates compliance with the treatment regimen.
Collaborates with members of the health care team around matters related to patient care. Collaborates with other social workers at Strong Memorial Hospital to ensure continuity of social work care between clinical settings. Participates in GCH@S discharge planning meetings to ensure continuous care needs are met.
Documents social work activity in the patient's medical record in accordance Social Work Division Policy and Procedure and CARES Peds Complex Care Program Policy and Procedure.
Serves as liaison to health and social service agencies involved in the care and support of patients and families. May participate as a group leader for support groups run by community organizations for CARES Center – Complex Care patients and families. May serve as a member of a Social Work Division committee.May serve a social work student intern supervisorMay participate in presentations to professional organizations and conferences.
Completes administrative requirements of the Social Work Division including, planned and unplanned absence process, productivity data other Hospital or Division forms; Hospital and Division mandatory in-service training programs, and UHS reviews.
Participates in professional development with particular focus on pediatric medical complexities, OPWDD system and services, child welfare, and pediatric to adult transitions.

QUALIFICATIONS:
The CARES – Peds Complex Care Social Worker must hold a Master of Social Work degree from an accredited institution. New York State licensure (LMSW) is required within eighteen months of employment required.
Experience in a pediatric setting with a multidisciplinary team is preferred.
Knowledge of OPWDD, OCFS and OMH systems .
Comfortable in making home visits on occasion preferred.
