POSITION TITLE: SOCIAL WORKER PRIMARY EVALUATOR PART-TIME (.5 FTE)
DEPARTMENT: STRONG BEHAVIORAL HEALTH
CLINICAL AREA: COMPREHENSIVE PSYCHIATRY EMERGENCY PROGRAM (CPEP)

I. GENERAL DESCRIPTION:
The social worker assigned to the Comprehensive Psychiatric Emergency Program (CPEP) functions as a primary evaluator and as social worker as dictated by the needs of patients present in CPEP. Primary Evaluator functions include completion of the social work psychosocial risk evaluation and psychiatric evaluation. The social worker also provides crisis intervention and ensures continuity of care between CPEP and inpatient or outpatient providers, resource consultation and routine discharge planning for patients they evaluate. Additional functions include: identification and intervention around individual and systemic barriers to discharge; collaboration with other agencies, services, community providers as needed towards achieving discharge; notification and involvement of administrative personnel as needed to achieve a safe discharge.
II. SPECIFIC RESPONSIBILITIES:
A. Administrative (10%)
E 1. Ensures that Social Work Division, Department, and Hospital policies and procedures are implemented with regard to professional practice, discharge planning and patient care.
E 2. Attends monthly Social Work Division and Program Staff Meetings.
E 3. Participates in routine committee process of the Social Work Division and complies with Social Work Division and Departmental quality assurance and staff development activities.
E 4. Participates in activities of the Social Work Division including, but not limited to timely completion of time reporting and other Hospital or Division forms; Hospital and Division mandatory in-service training programs and UHS reviews.
E 5. Subscribes to the Departmental goal of creating a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences.
E 6. Participates in available cultural competence events and training appropriate to job duties.
B. Clinical (90%)
E 1. Provides psychosocial risk assessment for patients seen in the Psychiatric Emergency Department to identify patients who are considered to be at elevated psychosocial risk.
E 2. Provides psychiatric evaluation and psychosocial assessment for patients seen in the CPEP to facilitate identification of patients who are in need of an inpatient psychiatric admission.
E 3. Provides clinical, collateral and psychosocial information to CPEP Provider for patients seen in CPEP, to be used in disposition determination by Provider.
E 4. Provides follow-up to patients and their significant support systems to promote continuity of care in situations where additional contact is needed to complete Emergency Department interventions such as crisis intervention, safety planning or to facilitate referrals for ongoing treatment and safe discharge.
E 5. Develops and maintains collaborative relationships with a variety of community and hospital resources who are involved in the care of patients or who refer patients to the CPEP.
E 6. Provides interventions to patients identified as having complex disposition/discharge planning needs by non social work evaluators.
E 7. Identifies specific individual and systemic barriers to discharge and develops a plan for addressing each barrier to affect a safe discharge in the most timely manner possible.
E 8. Facilitates involvement of agencies providing care to the patient and family/support networks, in working collaboratively for discharge of the patient to the community.
E 9. Notifies and involves supervisor and/or department administration as appropriate in order to effect a timely, safe discharge for patient.
E 10. Initiates referrals to departmental clinics and/or community agencies for service as needed to facilitate discharge.
E 11. Insures that proper treatment and referral protocols are followed with respect to child abuse, domestic violence, consultation with specialty clinics, and referral both within and outside of Strong Memorial Hospital.
E 12. Documents social work intervention in patient's chart in compliance with Division, Department and Hospital policy.
C. Teaching (TBD)
NE Based on experience and as determined by the social work supervisor may supervise social work graduate students, psychiatric residents, and medical students, assigned to the Psychiatric Emergency Program. Supervision relates to psychiatric/psychosocial assessment, crisis intervention, continuity of care, and disposition planning.
D. Research (TBD)
NE The social worker may participate in social work and or interdisciplinary research endeavors as determined by the social work supervisor, in consultation with the Social Work Clinical Manager in Psychiatry.
Ill. PROFESSIONAL SUPERVISION, ACCOUNTABILITY, AND EVALUATION:
The social worker's professional performance is supervised by a Social Work Division supervisor who is licensed by the State of New York (LMSW).
The social worker is accountable administratively and clinically to his/her social work supervisor (and through him/her to the Social Work Division Clinical Manager in Psychiatry) for provision of social work services, and adherence to Social Work policies, procedures, and standards of professional conduct. The social worker's job performance must conform to the expectations set forth in the position description.
The social worker is accountable to the Program and Medical Directors of the CPEP for providing professional social work services in accordance with the policies and standards established by the Program and the Department of Psychiatry.
Social work evaluations are conducted annually by the social worker's supervisor, with commentary from the Program or Medical Director of the CPEP. Promotional recommendations are made by the social worker's supervisor in consultation with the appropriate Social Work Division personnel. Salary increases and professional recommendations are subject to guidelines established by the Social Work Division, Strong Memorial Hospital and the University of Rochester.
IV. POSITION QUALIFICATIONS:
The social worker who occupies this position must have a MSW from an accredited social work graduate program and must obtain New York State licensure within eighteen months of employment.
V. POSITION DESCRIPTION MAINTENANCE:
This position description is reviewed annually by the social worker and his/her supervisor. Position review provides opportunity for comment from the Program or Medical Director of the Program and for updating, as needed. Similarly, if a new social worker is hired for the position, a review and update of the position description should occur. Changes in the position description are recommended by the supervisor, in consultation with the Program or Medical Director of the Program. Any position description modifications are authorized by the Social Work Division Clinical Manager for Psychiatry.
