  • General Description

    The Social Worker is responsible for providing clinical social work services to patients admitted to his/her designated medical/surgical unit (6-3400) and to their families or significant others, including adult patients admitted to the various surgical and cardiac services, and patients of all ages admitted to the unit under other clinical services. The Social Work functions as a member of the interdisciplinary health care team on 6-3400. The Social Worker serves as a liaison to health and social service agencies in the community.

    Specific Responsibilities

    A. Clinical

    Essential Functions 92.5%
  • The Social Worker:

  • Completes risk screening of all patients admitted to his/her designated unit.

  • Completes risk evaluations on patients identified as being at psychosocial risk.

  • Completes psychosocial assessments on patients requiring social work intervention.

  • Provides psychosocial counseling to patients and their families.

  • Serves as a resource consultant to patients and their families and facilitates referrals to appropriate community agencies and hospital resources.

  • Coordinates discharge planning for patients who are determined to be at psychosocial risk.

  • Collaborates with members of the 6-3400 health care team around matters related to patient care and discharge planning.

  • Handles risk management issues, in close consultation with the Social Work Division Director, Clinical Manager;

  • Documents social work activity in the patient's medical record;

  • Provides appropriate assessment and intervention services to children and adults suspected of being victims of abuse, neglect or domestic violence, and participates in relevant training and educational opportunities designed to enhance knowledge and skills in protective work;

  • Performs other duties, as assigned.

    Non-essential Functions

  • May make home visits, where appropriate, to provide essential psychosocial assessments and social work interventions;

  • Provides social work coverage, as requested, to other clinical units or programs during co-workers absence;

    B. Administrative

    Essential Functions 7.5%

    The Social Worker:
  • Completes Hospital and Social Work Division required in-service training programs within the required timeframe.

  • Completes required University Health Service appointments within the required timeframe.

  • Completes Social Work Division Daily Service Statistics , Time Distribution Sheets and other required forms and documentation within the designated deadlines.

  • Attends Social Work Division staff meetings.

  • Participates in professional development activities.

  • The social worker is expected to demonstrate the Strong Commitment – “I Care” values.

    Non-essential Functions

  • Serves as a member of the Social Work Division committees.

  • Serves as a liaison to health and social service agencies involved in the care of surgery patients.

    C. Supervision and Teaching

    Non-essential Functions

    The Social Worker:
  • May participate in the orientation and clinical teaching of surgical and nursing students, residents or staff.

  • May supervise BSW/MSW social work students.

    Professional Accountability, Supervision and Evaluation

    A. Supervision

    The Social Worker professional performance is supervised by a Social Work Division supervisor who is licensed by the State of New York (LMSW).

    B. Accountability

  • The Social Worker is accountable administratively and clinically to his/her social work supervisor and through him/her to the Director of Social Work and Patient & Family Services for the provision of social work services and adherence to Social Work and Hospital policies procedures and standards of professional conduct. The Social Worker's job performance must conform to the expectations set forth in the position description.

  • The Social Worker is accountable to the Chairman of the Department of Surgery and Medicine for providing professional social work services in accordance with the policies, and standards established by the Department of Surgery and Medicine.

  • The social worker is expected to demonstrate the Strong Commitment – “I Care” values.

    C. Evaluation

    Social Work evaluations are conducted initially at six months and annually thereafter by the Social Worker's supervisor, with commentary from other relevant faculty within the surgical departments or divisions. Salary and promotional recommendations are made by the Social Worker's supervisor, in consultation with the Director of Social Work, and must conform to the guidelines established by the Social Work Division, Strong Memorial Hospital and the University of Rochester .

    Position Qualifications

    The Social Worker who occupies this position must have a MSW from an accredited School of Social Work . The Social worker must obtain New York State License (LMSW) within eighteen months of employment.

    Position Description Maintenance

    The position description is reviewed annually by the social work and his/her supervisor. Position review provides opportunity for comment from appropriate Department of Surgery/Medicine faculty, and for updating, as needed. The position description is also reviewed and updated at the time that a new Social Worker is hired for the position. Changes in the position description are recommended by the supervisor. Position description modifications must be authorized by the Social Work Division Clinical Chief for Medical/Surgical Services.
