GENERAL DESCITPTION

The social worker is an integral member of the UR primary care site team. He/she provides screening, psychosocial assessment, psychiatric diagnosis and brief, evidence-based treatment for a primary care setting. In this role the social worker also helps to identify high risk patients with behavioral health and medical comorbidities and contributes to a unified person-centered treatment plan with the goal of improving the patient’s overall health and well-being, as well as facilitates access to specialty psychiatric and substance use care. The social worker participates in both formal and informal collaboration with primary care physicians, nurse practitioners and nursing care managers.

SPECIFIC RESPONSIBILITIES:

Administrative (5%)

  • Participates in the development of a supervisory contract and in annual performance evaluations.

  • Attends Social Work Division, Program Staff and Team Meetings as indicated

  • Ensures that Social Work Division and Hospital policies and procedures are implemented with regard to professional practice and patient care.

  • Participates in the routine committee process of the Social Work Division as appropriate, and complies with Social Work Division quality assurance and staff development activities.

  • Participates in activities of the Social Work Division including timely completion of time reporting and other Division forms; Hospital and Division mandatory in-service training programs and UHS reviews.

  • Subscribes to the UR Medicine goal of creating a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences.

  • Provides child protective consultation to primary care clinicians.

  • Participates in available cultural competency events and training appropriate to job duties.

    Clinical (95%)

  • As a member of the primary care team works with PCP’s to mutually enhance understanding of the patient/family, provides decision support for treatment planning and assists in the creation, implementation and monitoring of biopsychosocial treatment plans.

  • Performs diagnostic and functional evaluations and makes recommendations to the primary care provider concerning the patient’s treatment plan and goals.

  • Supports efforts for 100% behavioral health screening for depression, anxiety and substance use.

  • Offers brief interventions with patients including functional and strength-based assessment and diagnosis; psychoeducation for patients and their support systems; medication adherence; evidenced based treatments such as motivational interviewing; and brief problem-solving interventions aimed at promoting self-efficacy, enhanced decision making, and behavior change.

  • Provides Crisis Intervention during working hours.

  • Facilities referrals to specialty behavioral health care; wellness programs; and community agencies.

  • Advises primary care provider about which patients should be referred to a specialty mental health facility.

  • Initiates follow up to ascertain how patients are doing and to determine if changes in the treatment approach are indicated.

  • Assists in tracking patients’ adherence and engagement.

  • Documents treatment in patient’s electronic medical record in compliance with Division and Hospital policy.

  • Assists in tracking and monitoring high risk patients, including tracking patient’s adherence to treatment plans, reported outcomes tools, and status after referral to specialty mental health.

  • Collaborates with primary care providers.

  • Participates as a member of the clinical primary care team and attends meetings.

  • Provides and participates in clinician/staff training and education in integrated behavioral health primary care model.

  • Provides back up social work services to other collaborative care models during the absence of certain co-workers.

    Teaching

  • The social worker may provide supervision and training to social work students as determined by the social work supervision in consultation with the Clinical Manager for Social Work and Medical Site Director.

  • The Social worker may participate in teaching concerning patient and family assessment, and treatment interventions as they relate to the psychosocial aspects or behavioral health care of patients care within primary care setting.

    Research

    The social worker may participate in interdisciplinary and/ or social work research endeavors as determined by the social work supervisor in consultation with the Clinical Manager for Social Worker and UR Site Medical Director.

    PROFESSIONAL SUPERVISION, ACCOUNTABILITY, AND EVALUATION:

    Supervision/ Consultation

    The social worker’s professional performance is supervised by a Social Work Division supervisor who is licensed by the State of New York (LCSW-R).

    Accountability

  • The social worker is accountable administratively and clinically to his/ her social worker supervisor (and through him/her to the Social Work Division Clinical Manager in Psychiatry) for provision of social work services, and adherence to Social work policies, procedures and standards of professional conduct. The social worker’s job performance must conform to the expectations set forth in the position description.

  • The social worker is accountable to the UR site Medical Director and the Director of Collaborative Care and Wellness Division for daily clinical and operational matters and for providing professional social work services in accordance with the policies and standards established by the Primary Care Office Management Team and the Division of Social Work.

    Evaluation

    Social work evaluations are conducted by the social worker’s supervisor, with commentary from the UR Medical Site Director and the Director of Collaborative Care and Wellness. Salary and promotional recommendations are made by the social worker’s supervisor in consultation with the appropriate Social Work Division personnel. Salary increases and promotional recommendations are subject to the guidelines established by the Social Work Division, Strong Memorial Hospital and the University of Rochester.

    POSITION QUALIFICATIONS:

    The social worker that occupies this position has a MSW from an accredited social work graduate program. They must be licensed by the State of New York (LCSW) and competency in providing cognitive and behavioral interventions to individuals, families, and groups. Training and clinical experience in primary care behavioral health collaboration preferred. LCSW-R preferred.

    POSITION DESCRIPTION MAINTENCE:

    This position description is reviewed annually by the social worker and his/ her supervisor. Position review provides opportunity for comment from the UR site Medical Director and Director of Collaborative Care and Wellness and for updating as needed. Similarly, if a new social worker is hired for the position, a review and update of the position description should occur. Changes in the position description are recommended by the supervisor, in consultation with the UR Site Medical Director and Director of Collaborative Care and Wellness. Any position description modifications are authorized by the Social Worker Division Clinical Manager for Psychiatry.
