GENERAL DESCRIPTION :

The social worker assigned to the Strong Ties Clinic is responsible for providing immediate psychosocial assessment and social work treatment to individuals ages 18 and older who are severely and persistently mentally ill. This is provided through participation on an interdisciplinary team designed to provide comprehensive community based psychiatric care. The social worker functions as a crisis intervention worker for patients enrolled at Strong Ties and provides direct clinical service including individual, family and/or group therapy, crisis intervention, case management, and community outreach. The social worker may collaborate in policy and program development, operations managing, and plan program activities for the Strong Ties Clinic.

SPECIFIC RESPONSIBILITIES :

Administrative (5%)

  • Attends Social Work Division, Social Work in Psychiatry and Program staff meetings.

  • Ensures that Social Work Division, Department, and Hospital policies and procedures are implemented with regard to professional practice and patient care.

  • Participates in routine committee process of the Social Work Division, and facilitates Division and Hospital quality assurance and staff development activities.

  • Participates in activities of the Social Work Division including, but not limited to timely completion of time reporting and other Hospital or Division forms; Hospital and Division mandatory in-service training programs and UHS reviews.

  • Subscribes to the Departmental goal of creating a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences.

  • Participates in available cultural competency events and training appropriate to job duties

    Clinical (95%)

  • Participates as a member of an interdisciplinary treatment team involving consultation, psychosocial diagnosis, treatment planning, and periodic care review.

  • Provides billable crisis intervention services to patients enrolled at Strong Ties Clinic and collaborates with current treatment providers to ensure appropriate clinical care. This service may be provided in the clinic setting or in the community based on individual patient need.

  • May provide billable group therapy services to appropriate patients according to departmental policy.

  • Patient care also involves indirect case management services including referral to or liaison to other health and social agencies in the community.

  • Documents treatment in patient’s electronic medical record in compliance with Division, Department, and Hospital Policy.

    Teaching

    Based on experience and as determined by the social work supervisor, the social worker may supervise social work students. Upon invitation from the Program Director and in consultation with the social work supervisor, he/she may offer learning experiences for psychiatric residence, medical students, and family medicine residence. Supervision relates to psychosocial assessment and intervention with patients and their families.

    Research

    Based on experience/interest, may participate in interdisciplinary research as determined by the Social Work Supervisor, in consultation with the Social Work Clinical Manager in Psychiatry.

    PROFESSIONAL SUPERVISION, ACCOUNTABILITY, AND EVALUATION :

    Supervision/Consultation

    The social worker's professional performance is supervised by a Social Work Division supervisor who is licensed by the State of New York (LCSW).

    Accountability

    The social worker is accountable clinically and administratively to his/her social work supervisor (and through him/her to the Strong Ties Clinical Coordinator and Social Work Division Clinical Manager in Psychiatry) for provision of social work services, and adherence to Social Work policies, procedures, and standards of professional conduct. The social worker's job performance must conform to the expectations set forth in the position description.

    The social worker is accountable to the Director of Strong Ties Clinic for providing professional social work services in accordance with the policies and standards established by the Service and the Department of Psychiatry.

    Evaluation

    Social work evaluations are conducted annually by the social worker's supervisor with commentary from Strong Ties Social Work Clinical Coordinator as well as Strong Ties Director and/or designee. Annual salary and promotional recommendations are made by the social worker's supervisor in consultation with the appropriate Social Work Division personnel. Salary increases and professional promotional recommendations are subject to guidelines established by the Social Work Division, Strong Memorial Hospital and the University of Rochester.

    POSITION QUALIFICATIONS :

    The social worker who occupies this position must have a MSW from an accredited social work graduate program and a New York State license within eighteen months of employment. This position also requires experience in a mental health setting, preferably with the severely and persistently mentally ill population.

    POSITION DESCRIPTION MAINTENANCE :

    This position description is reviewed annually by the social worker and his/her supervisor. Position review provides opportunity for comment from the Clinic Director, and for updating, as needed. Similarly, if a new social worker is hired for the position, a review and update of the position description should occur. Changes in the position description are recommended by the supervisor, in consultation with the Director of the Clinic. Any position modification is authorized by the Social Work Division Clinical Manager for Psychiatry.
