The social worker is responsible for providing clinical social work services to adult patients admitted to the Intensive Care Unit and their families or significant others. The social worker provides crisis intervention, supportive counseling and concrete services to families of patients on the ICU. The social worker coordinates discharge planning for patients who are discharged directly from the ICU and may initiate discharge planning for other patients prior to their transfer to a regular medical or surgical unit. The social worker notifies appropriate inpatient social workers of patients who are being transferred from the ICU who are at psychosocial risk and ensures continuity of social work services upon transfer. The social worker serves as a member of the interdisciplinary health care teams in the ICU. The social worker serves as a liaison to health and social service agencies in the community. The social worker completes transplant evaluations on indicated patients while they are in ICU. Participates in liver transplant committee meeting as indicated.

Specific Responsibilities

Clinical: 80%

The social worker:

  1. Completes risk screening on all patients in the ICU through chart review and interdisciplinary rounds;

  2. Completes psychosocial assessments on patients requiring social work intervention.

  3. Provides psychosocial counseling to patients and families, including crisis intervention, supportive counseling and grief and bereavement counseling.

  4. Serves as a resource consultant to patients and their families and facilitates referrals to appropriate community agencies and hospital resources;

  5. Receives, prioritizes, and responds to referrals;

  6. Provides patients and families with appropriate educational and resource information and facilitates referrals to hospital resources and/or community agencies, as indicated.

  7. In collaboration with RN care co-ordinaters, assists with discharge planning for patients being discharged directly from the ICU.

  8. Collaborates with members of the ICU health care teams around matters related to patient care and discharge planning, when necessary;

  9. Collaborates with other social workers in the timely transition of care from the ICU to other units within the hospital, or vice versa.

  10. Handles risk management issues, in close consultation with his/her supervisor and the Clinical Manager;

  11. Documents social work activity in the patient’s medical record.

  12. Provides appropriate assessment and intervention services to children and adults suspected of being victims of abuse, neglect or domestic violence and participates in relevant training and educational opportunities designed to enhance knowledge and skills in protective work.

  13. Performs other duties, as assigned.

Non-essential Functions

  1. May make home visits, where appropriate, to provide essential psychosocial assessments and social work interventions.

  2. Provides social work coverage, as requested, to other clinical units or programs in co-workers absence.

Administration

Essential Functions

The social worker:

  1. Completes Hospital and Social Work Division required inservice training programs within the required time frames.

  2. Completes required University Health Service appointments within the required time frame.

  3. Completes Social Work Division Daily time entry in eRecord and other required forms and documentation within the designated deadlines.

  4. Attends Social Work Division staff meetings.

  5. Participates in professional development activities.

Non-essential Functions

  1. Serves as a member of designated Social Work Division committees and represents the Division on designated committees within the Hospital.

  2. Serves as a liaison to community health and social service agencies.

Supervision and Teaching

Non-essential Functions

The social worker:

  1. May participate in the orientation and clinical teaching of medical, surgery and nursing students, residents or staff.

  2. May supervise BSW or MSW social work students.

Professional Accountability, Supervision and Evaluation

Supervision

The social worker’s professional performance is supervised by a Social Work Division supervisor who is certified by the State of New York (LMSW).

Accountability

  1. The social worker is accountable administratively and clinically to his/her social work supervisor and through him/her to the Director of Social Work and Patient and Family Services for provision of social work services and adherence to Division and Hospital policies and procedures and standards of professional conduct. The social worker’s job performance must conform to the expectations set forth in the position description.

  2. The social worker is accountable to the chairs of the Departments of Medicine and Surgery and to the Directors of the MICU and SICU and the Director of Critical Care for providing professional social work services in accordance with the policies and standards established by the Department of Medicine and Surgery.

  3. The social worker is expected to demonstrate the Strong Commitment – “I Care” values.

Evaluation

Social work evaluations are completed initially at six months, twelve months, and annually thereafter by the social workers supervisor, with commentary from the chairs of the Departments of Medicine and Surgery, other relevant faculty within the departments and units. Salary and promotional recommendations are made by the social worker’s supervisor, in consultation with the Director of Social Work and Patient and Family Services and the Clinical Manager, and must conform to the guidelines established by the Social Work Division, Strong Memorial Hospital and the University of Rochester.

Position Qualifications

The Social Worker who occupies this position must have an MSW from an accredited school of social work and must obtain a New York State Certification (LMSW) within eighteen months of employment. The Social Worker is responsible for maintaining current licensure in accordance with NYS Office of Professions guidelines.

Position Description Maintenance

This position description is reviewed annually by the Social Worker and his/her supervisor. Position review provides the opportunity for comment from appropriate Department of Medicine and Department of Surgery faculty, and for updating, as needed. The position description is also reviewed and updated at the time that a new social worker is hired for the position. Changes in the position are recommended by the supervisor. Position description modification must be authorized by the Director of Social Work and Patient & Family Services.

Pay Range

Pay Range: $ 57,595 - $ 80,600 Annually

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
