Social Worker (S)
The Social Worker is responsible for providing clinical services to children ages 0-18 as well as young adults who are followed by the Pediatric Rheumatology, Genetics and Ophthalmology services. Clinical services are also provided to the families of these patients. Services are provided to outpatients, and on occasion inpatients where continuity of social work care is needed.
Provides risk screening/evaluations of Patients followed in these Divisions seen in the ambulatory setting through chart audit and staff referral. Completes psychosocial assessments of patients identified as requiring social work interventions.
Provides supportive counseling around adjustment to illness and coping
Serves as a resource consultant to patient’s families including; financial counseling for Medicaid/insurance, transportation to appts and meals, comforts during extended clinic visits.
Makes referrals to Children’s health home or other care management agencies or social services as indicated.
Identifies and manages all cases of suspected child abuse and neglect in the identified patient population. Provides assistance to patients and families which facilitates compliance with the treatment regimen
Identifies and intervenes around risk management issues, communicating these issues as needed to the clinical manager in social work.
Collaborates with members of the health care team around matters related to patient care and enhancing outcomes as well as with other social workers at GCH to ensure continuity of SWK care between settings.
Documents social work activity in patient’s medical record in accordance with SWD policy and procedure
Other duties as assigned
Provides coverage to other social work coworkers as needed
Participates in activities of the Social Work Division including but not limited to designating time spent in the patient’s medical record, following absence approval process, participating in professional development and completion of other Hospital or Division forms; Hospital and Division Mandatory in-service training programs, and UHS reviews.
May serve on a Division Committee
May participate in teaching of medical, nursing and social work students/staff
Must hold a Master of Social Work degree from an accredited institution. NY State licensure (LMSW) is required within eighteen months of employment.
Experience in a Pediatric setting with a multidisciplinary team is preferred
or equivalent combination of education and experience preferred
Knowledge of Psychosocial assessments and family systems preferred

Pay Range: $ 57,595 - $ 80,600 Annually
The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
