The social worker provides clinical social work services to adult patients (and their families) followed by the Multidisciplinary Oncology Service in the Pluta Cancer Center, an outpatient setting. The social worker serves as a member of the interdisciplinary health care team and is a liaison with health and social service agencies in the community.
Specific Responsibilities
Clinical Service
Essential Functions
The social worker: 92.5%
Meets newly diagnosed cancer patients to provide community resource information and general support.
Completes risk screening and evaluation to patients and family members identified as being at psychosocial risk.
Completes a psychosocial assessment on all patients in need of social work intervention.
Provides social work support services and bereavement counseling to cancer patients and their families through individual, family and group modalities.
Serves as a resource consultant to patients and their families and facilitates referrals to appropriate community agencies and hospital resources.
Works in close collaboration with other members of the Multidisciplinary Oncology Service to develop interdisciplinary care plans and interventions.
Handles risk management issues, in close consultation with the social work supervisor, Director of Social Work and Clinical Manager.
Documents social work activity in the patient's medical record.
Provides appropriate assessment and intervention services to children and adults suspected of being victims of abuse, neglect or domestic violence, and participates in relevant training and educational opportunities designed to enhance knowledge and skills in protective work.
Serves as liaison with Community Cancer organizations and participates in community collaborations.
Represents Cancer Center for interviews pertaining to emotional support or coping with a diagnosis as needed, in consultation with supervisor.
Performs other duties, as assigned.
Non-essential Functions
In special circumstances, visits patients' homes or community agencies to implement care plans.
Provides social work coverage to other units or programs, as requested, during co-worker's absence.
May serve as a facilitator for onsite cancer support group.
Administrative
Essential Functions
The social worker: 7.5%
Completes Hospital and Social Work Division required in-service training programs within the required timeframe.
Completes required University Health Service appointments within the required timeframe.
Completes Social Work Division Daily statistics in eRecord, Time Distribution and other required forms of documentation within the designated deadlines.
Attends Social Work Division and Cancer Center SW staff meetings.
Participates in professional development activities.
Non-essential functions
Serves as a member of the Social Work Division committees.
Serves as a member of other hospital, community, social work and patient groups to develop and expand services to cancer patients.
Supervision and Teaching
Non-essential functions
The Social Worker:
May participate in training staff of all disciplines and volunteers regarding the function of the Pluta Cancer Center social worker and/or the psychosocial needs of patients/families.
May supervise BSW or MSW social work students;
Professional Accountability, Supervision and Evaluation
Supervision
The social worker's professional performance is supervised by a Social Work Division supervisor who is certified by the State of New York (LMSW).
Accountability
The social worker is accountable administratively and clinically to his/her social work supervisor and through him/her to the Director of Social Work for the provision of social work services and adherence to Social Work and Hospital policies procedures and standards of professional conduct. The social worker's job performance must conform to the expectations set forth in the position description.
In addition, the social worker is accountable to the Director of the Cancer Center for providing professional social work services in accordance with the policies, and standards established by the Cancer Center.
The social worker is expected to demonstrate the Strong Commitment –“I Care” values.
Evaluation
Social Work evaluations are conducted initially at six months and annually thereafter by the social worker's supervisor, with commentary from relevant interdisciplinary team members and faculty within the Cancer Center. Salary and promotional recommendations are made by the social worker's supervisor, in consultation with the Director of Social Work and must conform to the guidelines established by the Social Work Division, Strong Memorial Hospital and the University of Rochester.
Position Qualifications
The social worker who occupies this position must have an MSW from an accredited school of social work and must obtain and maintain New York State Certification (LMSW) within 18 months of employment.
Position Description Maintenance
The position description is reviewed annually by the social worker and his/her supervisor. Position review provides opportunity for comment from appropriate Cancer Center faculty, and for updating, as needed. The position description is also reviewed and updated at the time that a new social worker is hired for the position. Changes in the position description are recommended by the supervisor. Position description modifications must be authorized by the Director of Social Work and Patient & Family Services.
