Social Worker

General Purpose:

The Social Worker is responsible for providing clinical services to children ages 0-18 and adults who are followed by the Pediatric Ambulatory Cardiology Practice, as well as their families.


Provides risk screening/evaluations of cardiac patients in the ambulatory setting through chart audit and staff referral. Completes psychosocial assessments of patients identified as requiring social work interventions.

Provides social work treatment and supportive counseling to patients and families including; adjustment to diagnosis, progression of disease and short-term bereavement counseling to families who have experienced a pediatric death.

Serves as a resource consultant to patient’s families including; financial counseling for Medicaid/insurance, transportation to appts and meals, comforts during extended clinic visits, as well as appointments at outside Institutions for second opinions, procedures and/or follow up from previous care.

Provides pre-admission and post hospitalization planning for patients.

Collaborates with members of the health care team around matters related to patient care and enhancing outcomes as well as with other social workers at GCH to ensure continuity of SWK care between settings.

Identifies and intervenes around risk management issues, communicating these issues as needed to the clinical manager in social work.

Documents social work activity in patient’s medical record in accordance with SWD policy and procedure

Identifies and manages all cases of suspected child abuse and neglect in the identified patient population. Provides assistance to patients and families which facilitates compliance with the treatment regimen. Makes referrals to Children’s health home or other care management agencies or social services as indicated.

Provides back up coverage in the absence of other social workers as needed.

Other duties as assigned

Participates in activities of the Social Work Division including but not limited to designating time spent in the patients’ medical record, following absence approval process, participating in professional development and completion of other Hospital or Division forms; Hospital and Division Mandatory in-service training programs, and UHS reviews.

May serve on a Division Committee

May participate in teaching of medical, nursing and social work students/staff


Must hold a Master of Social Work degree from an accredited institution. NY State licensure is required within 18 months of employment

Awareness of SWK role within a medical, interdisciplinary team, and/ or serving members of the patient population described in this position

Strong clinical assessment and critical judgement skills. Capacity to work within a multidisciplinary health team. Strong written and verbal skills. Comfort in navigating and documenting within an electronic medical record system. Ability to engage and serve a diverse patient population (preferred) .

NYS licensure within 18 months

Pay Range

Pay Range: $58,510 - $80,600 Annually

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
