The Optimization Specialist will act as the project manager and team lead for select Optimization Team initiatives. This position is dedicated to improving AHP’s performance and will collaborate with internal and external partners to integrate solutions into practice, and extend the reach and depth of Performance Optimization across the network while supporting its day to day activities. The Optimization Specialist will work closely with community based practices to improve quality and utilization performance through the implementation of best practices, workflow development, and co-creation of solutions. This individual will work with the inter-disciplinary Optimization Team to develop a plan for understanding the current state of the network, brainstorming solutions, creating roll-out materials, and supporting adoption across the network.
The Optimization Specialist will also assist with data translation efforts to understand why changes in performance have occurred and where opportunities exist for optimizing performance, and monitor prior and current initiative performance.
Optimization Team Activities
Lead select Optimization Team quality improvement initiatives and support the Optimization Team process
Identify project stakeholders; determine their needs and expectations for project success
Provide regular project status information to team members, including attending required meetings and following up on action items
Develop training and educational materials for Optimization Team initiatives, in collaboration with subject matter experts
Coordinate Optimization Team resources in and out of practices
Establish and maintain collaborative relationships with care managers and practice resources to understand practice operations and integrate initiatives
Engage community based practices struggling to meet quality and utilization metrics to optimize processes and workflows
Develop continuous monitoring reporting for Optimization Team initiatives, and work closely with the Data Coordinator to regularly review data regarding sustainability of initiatives
Monitor performance on past and current AHP initiatives and proactively identify areas of opportunity
Monitor key data and reports to inform AHP’s quality and clinical initiatives
Facilitate targets and thresholds selection for Optimization Team initiatives to determine success and monitor sustainability, in conjunction with the Director of Performance Optimization, Medical Directors, and Data Coordinator
Identify patterns of performance across segments of our network, quality measures, and utilization
Use data to inform components of Optimization Team initiatives and to assess and develop practice engagement and outreach
Establish collaborative working relationships with AHP practice teams and care management staff focused on improving performance
Assess the root causes of performance issues at the practice level and work with practice and care management staff to develop solutions to attain higher level of performance
Support practice and care management staff efforts to understand and utilize data to drive sustained quality and utilization improvements
Collaborate with appropriate resources to create and implement clinical and IT workflows in community based practices
Follow up with practices and care management staff who have implemented initiatives to learn best practices including how to sustain them, understand challenges, and recommend solutions
Bachelor's Degree in Computer Sciences, Systems Analysis, Industrial Engineering, Business Administration or other quantitative field and 3 years of related experience in Industrial Engineering or Systems Analysis; or an equivalent combination of education and experience.
Master’s Degree, Lean six sigma experience, PMP certification and/ or proven process improvement experience is a plus in this role.
